Athletic Training Forms

This page provides links to commonly needed forms for athletes and their parents.

Freshmen and transfer athletes are required to turn in three items each year:

• New Student Physical Form
• HIPPA Form and Contact Form
• A photocopy of your primary insurance card.

Returning athletes are required to turn in three items each year:

• Returning Student Physical Form
• HIPPA Form and Contact Form
• A photocopy of your primary insurance card.

Please return all completed material to the Head Athletic Trainer at the address below.

This information may be faxed to the attention of Athletic Training.
