Enrollment -
Prior to entry into the program you will need to complete all enrollment forms including, but not limited to the following:

• Application Form

• Certificate of Child Health Examination

• Office Rolodex Cards

• Child Care Food Program Parent Application Form

• Tuition Agreement

• Signature Sheet:  Campus Policies
