Applicant must include ALL colleges, universities, and specialized institutions that he/she attended or is currently attending. Applicant must submit official transcripts from the high school from which he/she graduated and all postsecondary work attempted. Failure to do so will lead to immediate dismissal and repayment of financial aid.

Also, if applicable, list branch military service, standardized exams taken for college credit (CLEP, DANTES, USAFI), and non-collegiate courses that have recommendation for college credit from the American Council of Education. An official transcript will be required for all course work and for test scores. A copy of the DD-214 form (military) and other appropriate military records are necessary to complete evaluation of credit.
