Student Health Insurance

St. Ambrose University requires that all students show proof of health insurance.

For Spring 2010 proof of current coverage or proof of elective enrollment in a policy of your choice is due by Dec. 4, 2009 in order to avoid having charges added to your student account. Charges may be reversed providing proof of coverage is received by Jan. 19, 2010. On Jan. 20, 2010, any student who has not repsonded with proof of coverage will be enrolled in the Student Assurance Services, Inc. insurance policy for the spring term and these charges may not be reversed.

(complete this online form and elect to pay online with a credit card or have charges billed to your student account)

(print and complete the form and mail in with check payment)

NOTE: The Student Assurance Services insurance policy does not provide benefits for expenses resulting from injuries sustained while participating in the practice or play of interscholastic or intercollegiate sports, including the participation in any conditioning program for such sport, contest or competition.
