How to Get Your
Veterans Educational Benefits Started at SAU
- VA Form 22-1990
(Form 22-5490 for dependents): If you've never used your
VA educational benefits before, you'll need to fill this
form (which is available in our office) out and return it
to our office. Download
the PDF version of the form.
- VA Form 22-1995
(Form 22-5495 for dependents): This is the form you
must fill out to request a change of program or place of
training with the VA. This form applies to transfer
students that have received benefits at another
institution. Download
a PDF version of the form.
- We also require
you to bring in a copy of your DD 214 (discharge
paper) prior to any service through our office.
- If you are
eligible to receive Chapter 31 Veterans Affairs Vocational
Rehabilitation Program benefits, please call: Dave Farner
at 563/ 391-1051.
Veterans Students
Responsibilities
Veteran students at St. Ambrose have ongoing responsibilities
to Veterans Affairs. DVA and the Veterans Coordinator at St.
Ambrose closely monitor each veteran student's enrollment
status and academic program, as the student's entitlement is
based upon the number of credit hours for which the student is
enrolled. Veteran students at St. Ambrose must do the
following:
- Notify the
Department of Veterans Affairs any time you withdraw from
a class, change your schedule or drop out of school while
the term is still in progress.
- Notify the Records
and Registration Office as a courtesy if you decide to
"step out" for a term.
- Notify the Records
and Registration Office if you change your degree program
or major.
- Upon earning 90
credit hours, you must declare a major and notify the
Records and Registration Office.
- When your
educational benefits have terminated, notify the Records
and Registration Office.
- When you change
your address, notify the Records and Registration Office
and the DVA.
